2011—2025年河南省平顶山市梅毒流行特征及时空聚集性分析

目的 分析2011—2025年河南省平顶山市梅毒流行特征及时空聚集性,为本地区梅毒防控策略制定提供流行病学依据。方法 收集2011—2025年平顶山市梅毒网络报告病例资料,采用描述性流行病学方法分析病例时间、地区、人群及疾病分期分布;采用Joinpoint回归模型分析报告发病率时序变化趋势;采用空间自相关分析及时空扫描统计分析识别发病空间聚集特征。结果 2011—2025年平顶山市累计报告梅毒病例12 948例,年均报告发病率为21.58/10万。报告发病率总体呈先升后降再升的阶段性波动变化。病例以隐性梅毒为主,占75.86%;≥20岁年龄组均以隐性梅毒为主,且占比随年龄增加而升高。不同地区发病率存在差异,高发地区主要为舞钢市、叶县和石龙区。空间分析显示,在未校正检验水准下,2016年和2019年呈弱正向全局空间自相关,经多重比较校正后,各年份全局空间自相关均无统计学意义;局部空间自相关和时空扫描识别出多个高风险聚集区。结论 平顶山市梅毒疫情总体呈先升后降再升趋势。隐性梅毒为主要报告类型。病例主要集中于≥60岁人群、20~39岁女性及农民、家务和待业人群,部分地区存在时空聚集。应加强隐性梅毒早期筛查、重点人群干预和高风险地区精准防控。

Abstract:

Objective To analyze the epidemiological characteristics and spatiotemporal clustering of syphilis in Pingdingshan, Henan Province, from 2011 to 2025, and to provide evidence for syphilis prevention and control. Methods Data on reported syphilis cases in Pingdingshan from 2011 to 2025 were collected. Descriptive epidemiological methods were used to analyze the distribution of cases by time, region, population characteristics, and disease stage. Joinpoint regression was used to analyze trends in reported incidence. Spatial autocorrelation analysis and spatiotemporal scan statistics were used to identify spatial clustering patterns. Results A total of 12,948 syphilis cases were reported in Pingdingshan from 2011 to 2025, with an average annual reported incidence of 21.58 per 100,000 population. The reported incidence showed a phased pattern of initial increase, subsequent decline, and later increase. Latent syphilis was the predominant stage, accounting for 75.86% of all reported cases. Among individuals aged 20 years and above, latent syphilis was the main reported stage, and its proportion increased with age. Reported incidence varied across regions, with higher incidence mainly observed in Wugang City, Ye County, and Shilong District. Spatial analysis showed weak positive global spatial autocorrelation in 2016 and 2019 at the nominal significance level; however, global spatial autocorrelation was no longer statistically significant in any year after multiple comparison correction. Local spatial autocorrelation and spatiotemporal scan analysis identified several high-risk clusters. Conclusion The syphilis epidemic in Pingdingshan City generally showed a trend of rising, then falling, and then rising again. Latent syphilis was the predominant reported stage. Cases were mainly concentrated among people aged ≥60 years, women aged 20–39 years, and farmers, homemakers, and unemployed individuals. Spatiotemporal clustering was observed in some areas. Efforts should be strengthened to promote earlier detection of latent syphilis, targeted screening of key populations, and precise prevention and control in high-risk areas.
