感染性疾病是造成儿童死亡最主要的原因之一,有效评估儿童感染性疾病的病情对改善其预后具有重大意义。目前临床常用的辅助检查包括病原学检测和C反应蛋白、降钙素原等血液学标志物检测,但病原学培养与鉴定耗时较长、部分血液学标志物的特异性不足。可溶性尿激酶型纤溶酶原激活物受体(soluble urokinase plasminogen activator receptor, suPAR)是一种新型生物标志物,为表达于多种免疫活化细胞表面的尿激酶型纤溶酶原激活物受体的可溶形式,在炎症或感染的过程中可反映机体免疫系统的激活程度,在多种儿童感染性疾病的早期诊断、病情评估及治疗监测等方面具有一定的应用价值。本文对suPAR在儿童肺部感染、疟疾、脓毒症、尿路感染及结核病等多种感染性疾病中的应用进展进行综述,并对未来可能的研究方向作出展望。
Abstract:Infectious diseases are one of the leading causes of childhood mortality, and effective evaluation of pediatric infectious diseases is crucial for improving their prognosis. Currently, commonly used clinical auxiliary examinations include etiological detection and hematological markers such as C-reactive protein and procalcitonin. However, etiological culture and identification require a considerable amount of time, and some hematological markers lack sufficient specificity. Soluble urokinase plasminogen activator receptor(suPAR) is a novel biomarker, representing the soluble form of the urokinase plasminogen activator receptor expressed on the surface of various immune-activated cells. It reflects the activation level of the body's immune system during inflammation or infection, and has certain practical value in the early diagnosis, disease assessment, and treatment monitoring of various pediatric infectious diseases. This article reviews the progress in the application of suPAR in various infectious diseases among children, including pulmonary infections, malaria, sepsis, urinary tract infections, and tuberculosis, and provides insights into possible future research directions.
