白细胞介素1受体相关激酶4基因多态性与儿童社区获得性肺炎的关联研究

摘要:

目的 探讨白细胞介素1受体相关激酶-4(interleukin-1 receptor associated kinase 4, IRAK4)基因的单核苷酸多态性与儿童社区获得性肺炎(community-acquired pneumonia, CAP)的相关性。方法 前瞻性采用改良多重高温连接酶检测反应技术对375例CAP患儿和306例健康儿童的IRAK4基因的5个标签位点进行分型,采用Logistic回归分析各位点基因型以及单倍型与儿童CAP的相关性。结果 IRAK4基因rs1141168G/A位点多态性与儿童CAP易感性显著相关(P<0.05);IRAK4基因rs4251580C/T和rs4251513C/G位点基因型CC以及单倍型CCGC与CAP患儿发生喘息的风险相关(均P<0.05);携带GTGT单倍型的CAP患儿并发脓毒症的风险显著降低(P<0.05);未发现与儿童CAP严重程度显著关联的IRAK4基因多态性位点及单倍型(均P>0.05)。结论 IRAK4基因多态性与儿童CAP的易感性、喘息及脓毒症的发生风险相关,但与CAP患儿严重程度无关。

Abstract:

Objective To investigate the association of single nucleotide polymorphisms(SNPs) of interleukin-1 receptor associated kinase 4(IRAK4) gene with the community-acquired pneumonia(CAP) in children. Methods A total of 375 children with CAP and 306 healthy children were prospectively enrolled. Five tag single nucleotide polymorphisms of the IRAK4 gene were genotyped using the improved multiplex ligation detection reaction technique. Logistic regression analysis was performed to assess the association of genotypes and haplotypes of each locus with pediatric CAP. Results The polymorphism of the IRAK4 gene at rs1141168G/A locus was closely associated with the susceptibility to CAP in children(P<0.05). The genotype CC of rs4251580C/T and rs4251513C/G locus and haplotype CCGC were significantly associated with the risk of wheezing in children with CAP(all P<0.05). Haplotype GTGT significantly reduced the risk of sepsis in children with CAP(P<0.05). No polymorphic loci and haplotypes of IRAK4 gene were found to be significantly associated with the severity of CAP in children(all P>0.05). Conclusion The gene polymorphism of IRAK4 gene is significantly associated with susceptibility, wheezing and the risk of sepsis in CAP children, but not with severity in CAP children.
