目的 探究结核性胸膜炎患者干扰素γ诱导蛋白-10(interferon-gamma inducible protein 10, IP-10)、辅助性T细胞1(T helper 1, Th1)/辅助性T细胞2(T helper 2, Th2)比值、Th1/Th2比值动态变化及其与胸膜厚度、胸膜粘连的相关性。方法 选取2018年1月至2023年1月在焦作市第三人民医院接受治疗(强化期2个月,巩固期10个月)的106例结核性胸膜炎患者为研究对象,治疗3个月后按治疗效果(疗效)分为有效组(n=77)和无效组(n=29),比较2组患者一般资料,胸腔积液IP-10水平、Th1/Th2比值以及胸膜厚度、胸膜粘连情况,分析胸膜炎患者IP-10水平、Th1/Th2比值动态变化与胸膜厚度、胸膜粘连的相关性,以及胸膜厚度、胸膜粘连与疗效的相关性。结果 与有效组相比,无效组患者胸膜厚度更大,胸膜粘连人数占比更高(P均<0.05);其它资料差异无统计学意义(P均> 0.05)。核性胸膜炎患者胸膜厚度与胸膜粘连紧密相关(r=0.867, P <0.05),并且胸膜厚度、胸膜粘连与疗效存在负相关关系(胸膜厚度:r=-0.787;胸膜粘连:Phi系数=-0.621;P均<0.05)。治疗后1周、1个月、3个月,患者胸腔积液IP-10水平、Th1/Th2比值均与胸膜厚度、胸膜粘连存在负相关关系(P均<0.05)。结论 结核性胸膜炎患者胸腔积液IP-10水平、Th1/Th2比值与胸膜厚度、胸膜粘连紧密相关,并与临床疗效相关,治疗3个月后相关性最高。
Abstract:Objective To investigate the changes in interferon-gamma inducible protein 10(IP-10), T helper 1(Th1)/Thelper 2(Th2), Th1/Th2 ratio dynamics in patients with tuberculous pleurisy and their correlation with pleural thickness and pleural adhesion. Methods One hundred six patients with tuberculous pleurisy who received treatment(intensive period of 2 months and consolidation period of 10 months) at Jiaozuo Third People's Hospital from January 2018 to January 2023 were selected as the study subjects, and they were divided into effective(n=77) and ineffective(n=29) groups according to the efficacy of the treatment after 3 months of treatment. The general data, pleural fluid IP-10, Th1/Th2 levels as well as pleural thickness and pleural adhesions were compared between the 2 groups to analyze the correlation between the dynamic changes of IP-10 and Th1/Th2 ratio and pleural thickness and pleural adhesions in patients with pleurisy, as well as the correlation between pleural thickness and pleural adhesions and the efficacy of treatment. Results Compared with the effective group, patients in the ineffective group had greater pleural thickness(P <0.05) and a higher proportion of pleural adhesions(P <0.05). No significant differences were observed in other data(P >0.05). Pleural thickness was strongly correlated with pleural adhesions in patients with tuberculous pleurisy(r=0.867, P<0.05), and there was a negative correlation between pleural thickness, pleural adhesions, and efficacy(pleural thickness: r=-0.787; pleural adhesions: Phi coefficient=-0.621; P <0.05). At 1 week, 1 month, and 3 months after treatment, the levels of IP-10 and Th1/Th2 in pleural effusion were negatively correlated with pleural thickness and pleural adhesions(P<0.05).Conclusion The levels of IP-10 and Th1/Th2 in pleural effusion in patients with tuberculous pleurisy are closely related to pleural thickness and pleural adhesions, and they are also correlated with clinical efficacy. The correlation is highest three months after treatment.
